Approach

One method, from a single agent to an enterprise programme

Every engagement asks the same three questions, examines the same ten assurance areas, and ends in a decision that leadership can act on. Only the scope changes.

The three questions

What leadership actually needs to know

01 · Safe

Is it safe?

Can it be trusted not to cause harm or break under pressure?

  • Safety — resistance to harmful, disallowed or dangerous output
  • Security & adversarial resistance — jailbreaks, prompt injection, deliberate misuse
  • Robustness — messy, edge-case and out-of-distribution input
  • Privacy & data handling — does it leak or mishandle sensitive data?
02 · Skilled

Is it a skill?

Does it actually do the job — on real tasks, not a scripted demo?

  • Accuracy and capability on tasks drawn from your domain
  • Task completion — end to end, does it finish the job correctly?
  • Reasoning & tool use — for agents, every step and tool call, not just the answer
  • Consistency — same quality across repeated and varied runs
03 · ROI-Fit

Is it working as per my investment?

Is it worth what it costs?

  • Business value — does the output move the metric it is supposed to?
  • Reliability in production — failure rate, recovery, human-handoff need
  • Cost & latency — tokens, time and money per successful outcome
  • Fit to claim — does real performance match what was sold?

Graded separately — never blended into one number that could hide a serious gap.

Ten assurance areas

Where evidence is examined

01Governance & accountability
02Use case & risk classification
03Data governance & privacy
04Model & supplier management
05Evaluation & testing
06Security & adversarial robustness
07Agent controls
08Transparency & disclosure
09Monitoring, incidents & change
10Value & performance
The evidence scale

Intent is not evidence

Every control is rated on what can be proven, not what is intended. “Practised but not documented” is treated as a gap, because that is how an auditor or a buyer will read it.

  1. Documented & evidencedWe could hand over proof today
  2. Practised, not documentedWe do it, but couldn't show evidence
  3. PlannedOn the roadmap, not yet in place
  4. Not in place
  5. Don't know
For AI agents

The agent loop

An agent does not just answer — it acts. We test every stage.

01

Understand

Interprets the request, the goal and the constraints.

We test: Ambiguous, adversarial and out-of-scope requests.
02

Observe

Gathers context: conversation, permissions, system state.

We test: Missing, stale or manipulated context — including hidden instructions.
03

Locate

Finds the right data, document or tool for the next step.

We test: Wrong-record retrieval, permission leaks, tool selection.
04

Act

Calls the tool, writes the record, sends the message.

We test: Irreversible actions, argument correctness, least privilege.
05

Verify

Checks the result; recovers or hands over when it fails.

We test: Tool failures, silent errors, loops and escalation behaviour.
The decision

Every engagement ends in a release gate

Go

Every dimension Good or better, no open critical issue, major issues within agreed limits.

Conditional

No open critical issue; named issues fixed and re-tested before scale-up.

No-Go

Any open critical issue, or any dimension graded Poor. Blocked until re-evaluated.

ROI-Fit

The expensive part is the gap between the claim and reality

If the vendor says 5% errors and your tasks produce 12%, the business case is wrong from day one. Try your own numbers.

Unbudgeted cost of the gap
$672K / year

1,400 more bad outcomes every month than the business case assumed.

# shown with its working
20,000 × 12 × (12% − 5%) × $40
= $672K

An independent evaluation measures the real rate before you commit, rather than after the invoices arrive.
